Below you will find useful information about the 2012-2013 Claremont Colleges Student Health Insurance Plan (SHIP).

ID CARD

Your permanent ID card will be mailed to you shortly after the start of the term. If you need to seek treatment before you receive your permanent ID card, you can dowload a temporary ID Card by clicking here.

Please allow at least 5 days after the term begins for your information to be available.

ATTENTION
As of September 15, 2012, Walgreens will begin to participate in the Express Scripts retail pharmacy network.

ENROLL IN SHIP

If you are a student at Harvey Mudd College, please complete this HARVEY MUDD ENROLLMENT FORM.

Otherwise, if you are one of the following students and would like to enroll in SHIP, you may download the STUDENT ENROLLMENT FORM here:

Claremont Graduate University Domestic Student
Keck Graduate Institute Domestic Student
Pomona College Domestic or International Student (NEW IN SPRING OR SUMMER ONLY)
Scripps College Domestic or International Student (OTHER THAN FIRST YEAR)

All other students are enrolled through the school and do NOT need to complete an enrollment form.

Note: Students enrolling a Domestic Partner in the plan must provide an Affidavit of Domestic Partnership to Ascension Benefits & Insurance Solutions along with the dependent enrollment form. Click here to download an affidavit.
